Choosing Wisely Home Health Care provides an array of services to meet the needs of our patients.

Our services include:  Home Health Care, Companion Care, Personal Care, Respite Care, and Private Duty Care. 

                                                            

We perform light house keeping duties, meal preparation, shopping, grooming to include hair, shaving, bathing assistance, bathroom assistance, medication assistance and non-skilled in home care services for illness and/or injury.         

                             

Choosing Wisely Home Health Care takes immaculate care of our patients, as we are providing care for them at a vulnerable time in their life. Their comfort level and safety is our top priority.

              

Whether it is helping them navigate through daily duties in their home, or sitting with them and providing comfort to them in a nursing facility, rehabilitation center, or the hospital, Choosing Wisely Home Health Care staff bring the compassion and experience in all settings.

Our office hours are Monday through Thursday, from 10am until 3 pm.

                      

We provide care 24 hours per day, including holidays.

We provide care in a home setting, or medical setting once approved by the patient and/or the family for Private Duty care.
